Longevity Health is seeking an experienced On-Call Advanced Practice Provider (Nurse Practitioner) to deliver after-hours telehealth support for an I-SNP and long-term care population. This remote role covers multiple states (TX, FL, CO, MI, NC, SC) with licensure in active states and emphasis on treating in place and minimizing ED visits. You will perform acute assessments, order diagnostics, and coordinate care with facility teams, maintaining robust EMR documentation and patient-centered #J-18808-Ljbffr Longevity Health
