We are seeking a dedicated Patient Care Coordinator in the homecare industry. This role involves managing daily patient care and coordinating caregivers to ensure high-quality service and accurate recording of caregiver clock-ins and outs. Proficiency in programs like HHAx and experience with Smartsheet are required. The ideal candidate is motivated, patient, and proactive.
**Key Responsibilities:**
- Oversee daily patient care activities and caregiver scheduling.
- Communicate effectively with patients, families, and caregivers.
- Adjust care plans as necessary.
- Ensure compliance with healthcare regulations.
- Use HHAx for scheduling and tracking.

- Skilled in HHAx, Smartsheet, and computer use.
- Strong communication and problem-solving skills.
- Proficiency in English and Spanish.
