Humana Inc. is seeking a Case Manager to join the Inclusa/Humana team serving Wisconsin Family Care members in Eau Claire and surrounding areas. You will report to the Manager of Care Coaching and provide comprehensive case management for frail elders and adults with intellectual, developmental, or physical disabilities. Responsibilities include assessing health needs, developing MCPs, coordinating with a Field Care Nurse, and maintaining documentation. #J-18808-Ljbffr
