Position Overview The Columbia County Homeless Intervention Case Manager(“CCHI”) assists families and individuals with all facets of the intake process. This includes performing assessments, service plan development and coordinating housing activity with families. The Case Manager will also coordinate service provisions with the Family Care Coordination Program. Position Qualifications & Requirements High School Diploma / GED equivalent required; Experience in human services preferred; Associate/Bachelor’s Degree with limited experience. Professional record of integrity and strong advocacy for homeless families. Ability to work flexible hours to accommodate client schedules, which may include evening and/or weekend hours, including rotation of holidays. Local travel is required; A current Clean and Valid New York State Driver’s License (actively licensed at least one year) - is required *. Knowledge of community resources is beneficial. Previous experience working in a human services agency is a plus! Proficiency in computer applications: Microsoft Office Suite (Word, Excel), Google Business Workspace Suite (Gmail, Google Docs, Meet, Sheets, etc.); and secure Internet practices. Outstanding interpersonal and communication skills; Capable of building strong working relationships with regulatory bodies. Self-motivated and engaged, with a strong attention to detail; Excellent writing aptitude. A compassionate, warm and caring mindset; Capable of exhibiting empathy for those in crisis. Capacity and openness to work with populations from culturally diverse and economically disadvantaged backgrounds, including but not limited to a broad range of people with various developmental and functional levels. Qualities of positivity, flexibility and adaptability; Proven reliability and dependability. Capable of exercising ethical, independent judgment and discretion, handle sensitive data with strict confidentiality. *Staff in driving positions must be able to meet the agency’s minimum requirements of our insurance policy for authorized drivers; including, but not limited to the minimum age of 21 at time of hire, and the ability to maintain insurability throughout employment.* Essential Duties & Responsibilities include: Conduct and complete documentation for intake/assessment process, initial service plan, bi-weekly service plan updates, quarterly Critical Time Intervention Phase Goals, and coordination of service plan delivery through a large network of area service providers. Support and facilitate the individual’s ability to participate in community services, in particular, trauma services through Mental Health Providers, inpatient/outpatient treatment through Substance Abuse Providers, and specialist care for physical health concerns. Meet with families and individuals to assist them with service plan goals; follow up with families/individuals and /or service providers as needed to ensure service plans goals are being accomplished; act responsibly in providing information and empower residents to identify choices in their daily lives. Meet independently with families and individuals to assess needs and strengths. Establish and maintain professional working relationships with families and staff. Advocate for families and individuals; assist in obtaining necessary services to retain permanent housing and assist in identifying and resolving barriers to housing acquisition/retention. Act as liaison for families and individuals when communicating with public assistance and other resource providers. Provide intensive case management services to assist in settling family or individual and in retaining permanent housing, including stabilizing conditions that led to homelessness. Document interactions utilizing proper procedures; complete outcome measures for Quality Improvement; submit reports in a timely manner. Facilitate the location and acquisition of permanent housing with the individual/family via housing searches online or via street outreach and the completion of subsidized housing applications for Columbia/Greene and surrounding Counties. Assist with applying for furniture, security deposit, and first month’s rental assistance with partnering community agencies. Assist with moving individuals/families and their personal belongings from their temporary housing location to their established permanent housing location using agency vehicles. Work with the individual/family for one year working collaboratively on creating goals based on the Critical Time Intervention Phase Goal Model. Intervene in crisis situations and assist with conflict resolutions. Additional position-related duties and responsibilities as assigned by the supervisor.
