Location 245 West 129th St,New York, NY, 10035,United States Base Pay $21.77 - $21.77 / Hour Job Category Health Education and Access Coordinator Employee Type FT Non-Exempt Required Degree High school Description Under the Cancer Screening Navigation Programming, the Health Education and Access Coordinator will help assess individual and community health education needs related to Prostate Cancer; plan, implement, and evaluate effective health education programs; coordinate health education services; serve as a resource person in health education. The goal of the Health Education and Access Coordinator is to provide people who are at high risk of Prostate Cancer with tools that allow them to prevent developing life-threatening health issues. Responsibilities & Duties Health Education – Individual Make referrals to medical provider and social work for all new patients. Maintain chart documentation for each encounter; clinical care supervised by collaborating clinician. Conduct follow‑up and outreach with patients as indicated. Health Education – Classroom and Small Group Assess individual and community needs for health education on an ongoing basis. Work with Community Health Practicum Adolescent Health Educator to develop/select/adapt and implement a health curriculum to be delivered to students in the classroom and small group setting. Evaluate effectiveness of health education activities and revise as needed with implementation of current best practices. Work with SBHC staff on other tasks as needed. Insurance Access – Individual Screens children and families for Medicaid, CHP, FHP, FBPB, PCAP and WIC eligibility. Completes the Access NY application for eligible families. Provides intensive tracking and follow‑up with applicants to ensure that all required supporting documentation is obtained. Works to implement insurance related outreach strategies. Tracks reporting metrics with supervisors on SBHC billing and reimbursement to identify barriers to effective billing and problem‑solve/improve SBHC registration workflow.

QUALIFICIATIONS:

Bachelor’s degree in social work, psychology, public health, health education or related field. 1‑2 years of experience in health education and/or insurance access/enrollment. Demonstrated familiarity counseling adolescents on reproductive health (experience preferred). Demonstrated familiarity with health insurance issues. Able to manage competing demands and monitor multiple projects simultaneously. Very detail oriented and thorough. Able to work independently and as part of a small team.
