Nurse Practitioner or Physician Assistant

Practice medicine in a way that prioritizes time, connection, and meaningful patient relationships. As a Nurse Practitioner or Physician Assistant in Physician Services, you will provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with the autonomy to focus on patient‑centered care that improves comfort, function, and quality of life. In this role you will conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team members. You will serve as a key clinical decision‑maker and help patients and families better understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations. Location Office Location: Jackson, MI Main Service Area: Greater Jackson and surrounding areas This role is a high‑travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes.
