Work Environment

This full-time position primarily operates onsite, within the homes of our patients in Thousand Oaks and surrounding areas. Key Responsibilities

• Conduct thorough patient assessments, utilizing evidence-based practices to evaluate physical conditions and rehabilitation needs.
• Collaborate with physicians and the healthcare team to develop and implement individualized care plans aimed at achieving defined patient goals.
• Administer therapeutic interventions and exercises designed to improve mobility, strength, and functional abilities, closely monitoring patient progress.
• Educate patients, their families, and caregivers on at-home care techniques, therapeutic exercises, and the proper use of assistive devices or orthotics to facilitate an independent recovery journey.
• Pace your workload effectively by prioritizing caseloads, coordinating daily assignments, and managing hand-offs to maintain a seamless patient care environment.
• Maintain precise documentation of all interactions, treatment plans, and progress notes to ensure compliance with both legal and ethical standards of practice.

Patient Population / Client Focus

You will primarily serve a diverse clientele, including adults recovering from surgical procedures and children facing developmental challenges. Your role will require an understanding of different rehabilitation needs, which includes not only physical recovery but also addressing emotional and psychological aspects as patients navigate their healing processes.

Qualifications

• A Bachelor's Degree in Physical Therapy, with a valid California Physical Therapist License is required.
• New graduates are welcome; a passion for patient care and eagerness to learn are as valuable as experience.
• Basic Cardiac Life Support (BCLS) certification must be obtained within 30 days of starting employment.

Preferred Experience

While no prior experience is required, candidates with some familiarity with outpatient or home healthcare settings will be prioritized. Experience using patient management software for documentation and scheduling can enhance workflow efficiency and patient tracking.

Compensation and Benefits
