Team 8: Coram, Middle Island, Miller Place, Mount Sinai, Ridge, Rockypoint, Selden, Shoreham, Sound Beach, Upton, Wading River, Yaphank, North Fork , Aquebogue, Baiting Hollow, Calverton, Flanders, Jamesport, Laurel, Mattituck, Northville, & Riverhead. * 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends * Per visits are required to attend an entire first week of orientation Monday - Friday (8:30AM -4:30PM.) * Per Visits are required to work 1 shift per pay period. Job Details Our Home Care RN's: * Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients. * Measure vital signs, Administer IVs, change dressings, clean wounds as needed * Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. * Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload. * Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment. * Provide care to the most upmost standard Minimum Qualifications: * New York State RN license * Minimum of one year of recent hands on experience as a Registered Nurse. * Basic Life Support Certification * Valid New York State Driver's License and reliable transportation Salary Range USD $35.00 - USD $135.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
