Position Overview Job Title: Registered Nurse (RN) Location: Rochester, NY – Various Day Services Locations including Greece, Gates & Spencerport Department: Nursing Reports To: Linda Holmes, Director of Health Services Employment Type: Full-Time, Days Starting Wage: $74,287.32 - $82,823.64 annually, commensurate with experience and internal equity plus a $3,000 sign on bonus! Your Core Responsibilities Monitor and coordinate the health and well-being of individuals supported, ensuring timely follow-up care. Oversee and ensure compliance with all medication administration procedures, policies and inventories. Train, supervise and evaluate direct support staff and LPNs in nursing practices and medication protocols. Maintain accurate, timely medical documentation and electronic health records in compliance with regulations. Collaborate with families, advocates, physicians and team members to provide person-centered care. Respond to health emergencies, conduct clinical assessments and coordinate post-discharge care. Provide medical coverage across assigned sites and review RN on-call reports as needed. What You Bring Current, unencumbered NYS Registered Nurse (RN) license. At least 1 year of nursing experience. Strong clinical judgment, communication and documentation skills. Ability to work independently and collaboratively with interdisciplinary teams. Valid NYS driver’s license and reliable transportation. Experience supporting individuals with developmental disabilities preferred but not required.
