Inpatient Rehabilitation Clinical Liaison

Eastern Iowa Rehabilitation Hospital seeks a Clinical Liaison to connect patients, families, and referral sources with our inpatient rehab services. In this role, you will perform pre-admission screenings, evaluate rehab appropriateness, and coordinate smooth transitions of care. You will educate providers and the community on our programs, build and maintain referral relationships, and collaborate closely with an interdisciplinary rehab team.
