Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and monitoring patient conditions

• Develop and implement individualized care plans that address the unique needs and goals of each patient

• Collaborate with the interdisciplinary care team to ensure comprehensive care and coordination of services

• Document patient care and progress in accordance with facility policies and regulations

• Participate in quality improvement initiatives and contribute to the development of best practices

Requirements:

• Current LPN license in the state of Pennsylvania
