Pharmaceutical Care Coordinator

Duties

The role involves managing and coordinating comprehensive pharmaceutical care activities within the Michael E. DeBakey VA Medical Center and its community-based outpatient clinics. Key responsibilities include:

• Developing and directing clinical pharmacy programs in the Extended Care Line.
• Promoting and delivering collaborative, evidence-based, patient-centered, comprehensive medication management in an interprofessional direct patient care setting.
• Integrating clinical practice guidelines, The Joint Commission (TJC) standards, Veterans Affairs (VA) performance measures, and VA Pharmacy Benefits Management (PMB) initiatives into practice.
• Functioning as an advanced practice provider with prescribing privileges, working independently with minimal supervision.
• Operating the clinical pharmacy program in assigned areas, ensuring the safe, appropriate, cost-effective use of drugs.
• Providing accurate, up-to-date, relevant drug information and drug literature analysis to health care professionals.
• Participating in quality assurance programs and providing education programs for patients and staff.
• Supplying coverage to other clinical pharmacy areas as assigned.

Work Schedule: Monday - Friday: 8:00am - 4:30pm, subject to change based on the needs of the facility.
