Registered Nurse (RN) - Case Management

RN – Home Health

Schedule: Monday to Friday with weekend rotation approximately every 4th week

Assignment Details:

• Guaranteed Hours: 36
• Contract Length: 12 weeks

Requirements:

• Active RN license

Roles & Responsibilities:

• Provide individualized, goal-oriented nursing care following the nursing process in accordance with physician orders.
• Serve as a Case Manager when acting as the lead discipline in care coordination.
• Collaborate with interdisciplinary teams to enhance patient care.
• Maintain accurate documentation of patient care and treatment plans.
• Educate patients and families about health management and support services available.
• Address patient needs and ensure compliance with healthcare regulations.
