Home Care Field RN Supervisor – Per Diem Service Area: Monmouth County and select areas of the surrounding counties. Integral Care Home Care is expanding into Central Jersey. We are just getting started in this area and anticipate a growing number of new cases, with opportunities for additional work as we expand. Home care experience is required. Qualifications: Valid, current New Jersey Registered Nurse license Current driver’s license Organized and patient-focused Able to work independently Strong oral and written communication skills Minimum of 1–2 years of clinical nursing experience Responsibilities: Assess patients at the start of care and develop individualized plans of care. Update plans of care as patients’ needs change. Evaluate patients in their homes. Instruct and guide patients, families, and home health aides regarding patient care. Reassess patients every two months to identify changes in condition and supervise home health aides to ensure appropriate care. Once you begin working with us, come into our Freehold office to provide caregiver training, initially one day per week. Availability for weekly office training is required. Compensation and Benefits: Hourly rate: $50–$100 Initial assessments and reassessments: $100/hour After-hours: additional $25 Office time, including caregiver training: $50/hour HHA supervisory visits: $75/hour Weekend visits: $75/hour On-call: $50 per day when calls occur Mileage reimbursement #J-18808-Ljbffr Integral Care Home Care
