Position Overview: 

We are seeking an experienced and compassionate Wound Care Nurse to join our team. This role involves assessing and managing acute and chronic wounds, developing tailored treatment plans, and collaborating with a multidisciplinary team to ensure optimal patient care. Responsibilities include conducting evaluations, implementing evidence-based interventions, educating patients and families, and maintaining accurate documentation. • Conduct thorough assessments of patients' wounds to determine the appropriate treatment plan.

• Implement and monitor wound care treatments based on evidence-based practices and protocols.

• Educate patients and their families on wound care management, including prevention and home care techniques.

• Collaborate with physicians, nursing staff, and other healthcare professionals to develop and execute individualized care plans.

• Maintain accurate and up-to-date documentation of wound assessments, treatment plans, and progress notes in the patient's medical record.

• Participate in quality improvement initiatives related to wound care and ensure compliance with health regulations.

• Stay informed about the latest advancements in wound care and participate in ongoing professional development.

Requirements:

• Current Licensed Practical Nurse (LPN) license in the state of practice.

• Certification in Wound Care Nursing (CWCN) or equivalent preferred.

• Minimum of 2 years of nursing experience, with at least 1 year in wound care management.

• Strong knowledge of wound healing principles, assessment techniques, and treatment modalities.
