Registered Nurse – Home Health

Schedule: Monday–Friday, 8:00 AM–4:30 PM EST
On-Call: Every 3 weeks — $25/day and $50/weekend day/holiday
Coverage Area: Shelby County, TN

We are seeking a Registered Nurse (RN) to provide comprehensive, patient-centered care in a home health setting. The RN will assess patients, implement and evaluate care plans, coordinate with healthcare providers, and support health maintenance and disease prevention.

Responsibilities

• Perform patient assessments and develop, implement, and evaluate individualized care plans.
• Provide skilled nursing care and case management in the home setting.
• Complete required clinical documentation, including OASIS and nursing notes.
• Monitor patient conditions and communicate changes to physicians and other healthcare providers.
• Coordinate medications, physician orders, appointments, and follow-up care.
• Collaborate with nurses, physicians, and other healthcare professionals.
• Supervise LPNs and support staff as needed.
• Provide health and safety education to patients and caregivers.
• Maintain compliance with infection control, medication management, and patient safety standards.

Requirements

• Active Tennessee RN license ; compact licenses accepted.
• Home Health experience required.
• OASIS and HCHB experience required.
• Wound care competency preferred; training available if needed.
• Current BLS certification .
• T-Step TB test.
• Strong clinical, communication, and organizational skills.
