Stanford Health Care Tri-Valley Day - 10 Hour (United States of America) Brief Overview Assesses patients and establishes appropriate treatment plan based upon diagnosis and professional standards. Provides appropriate treatment of patients using specific modalities and treatment techniques. Accurately documents treatments and care of patients and communicates with other team members. Participates in individual and department continuous improvement and education activities. Locations Stanford Health Care Tri-Valley What You Will Do Direct patient care, including the assessment and treatment of patients, developing and carrying out individually designed treatment programs. Perform and document an initial evaluation, interpret data to identify problems, and determine functional limitations prior to intervention. Perform ongoing documentation in the patient medical record with functional limitations, goals and appropriate timelines. Communicate with referring physician to obtain certification of plan of care and document as appropriate. Include the patient/caregiver in treatment process, evaluation of effectiveness and discharge as necessary. Evaluate the effectiveness of treatment at various stages and make adjustments to achieve maximum benefit or discharge when treatment is no longer needed. Ensure patient education, proper fit and use for assistive devices as necessary. Supervise PTA, PSM Aide, students and volunteers as the clinical schedule dictates. Participate in team and hospital-wide meetings, committees and initiatives as assigned. Assist with department finances with accurate daily billing practices, active daily maintenance of patient care schedule. Perform all duties and responsibilities in accordance with the C-I-CARE Standards of the Hospital. Education Qualifications Graduation from a physical therapy program from an accredited college or university. Experience Qualifications 3+ years to 5 years of experience. Required Knowledge, Skills and Abilities Criteria to attain level 2 requires 3 years experience, with one year at SHC TV preferred; and 1 of the following 7 criteria be met at application. Certifications: Achievement and maintenance of relevant APTA board of PT specialties and AOTA specialty areas or ASHA specialties. (List of specialties omitted for brevity.) Hospital committee member: Regular member (80% attendance or more) of a committee that meets at least once per month. Student clinical instructor: Main clinical contact for a professional student or volunteer. Department champion for hospital or department initiative: Regular member with active participation. Assist with current and regular class or program within PSM department. Assist with formalized implementation of curriculum for new/current employees. Treat a sub-specialty population with required continuing education competence. Preferred Knowledge, Skills and Abilities Good physical condition, which requires moderate lifting of equipment and patients. Ability to work flexible schedules and varying locations as assigned. Strong verbal and written English communication skills. Strong organization and time management ability. Problem solver and critical thinker with the ability to anticipate possible issues. Strong computer skills. Self-initiative. Knowledge of Medicare documentation standards. Knowledge of hospital safety, quality and joint commission standards. Knowledge of standard medical abbreviations for documentation. Licenses and Certifications Current American Heart Association Certification for Basic Life Support for Healthcare Providers required. Physical Therapy in the State of California required (Codes: OTR, PT, SLP).
