Registered Nurse PositionHealthPartners is currently hiring for an RN at our Wabasha location.QualificationsRequired: Graduate of an accredited school of nursing Current RN licensure in Minnesota Current RN licensure in Wisconsin within two months of hire Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable Strongly demonstrated written and oral communication skillsPreferred: One year full time employment as an RNPhysical Requirements: Ability to sit or stand for prolonged periods at a time. Must possess speech, hearing and sight skills to perform the functions of the position. Manual dexterity required. Must be able to speak, read and write English proficiently. Planning is the process of making decisions and setting goals which are mutually acceptable to the nurse and all pertinent others. Implementation is the process of providing comprehensive nursing care. It assists patients/families to optimally manage their health care, respect their culture and values and helps to identify individual needs, health goals and treatment preferences.Performance Is Up To Standard When: Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention Identifies and secures patient services needed for patient care Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.EducationDefinition: Education imparts knowledge designed to develop skills, to enhance decision-making, and to promote understanding.
