Bilingual Referral Intake Specialist - In-Home Health
Esperta Health
Esperta Health is seeking an Intake Coordinator who will own referral intake after an initial member engagement. You will ensure referrals are complete, accurately entered, and routed to the right team, tracking them through admission or final disposition. Reporting to the Director of Clinical Operations, you will collaborate with members, caregivers, providers, health plans, and internal teams to resolve missing information and prevent delays.
