Occupational TherapistWe are looking for a Home Health Occupational Therapist (OT) to help patients build independence, safety, and functional ability after an injury, illness, or surgery.Responsibilities include:Functional Evaluations: Assess a patient's ability to complete daily tasks like bathing, dressing, grooming, and cooking.Home Safety Assessments: Inspect the home for fall risk, poor lighting, or tight spaces, and suggest changes like grab bars or ramps.Personalized Care Plans: Create clear, measurable therapy goals that fit the patient's home environment and medical needs.Caregiver Education: Train family members and hone health aides on safe lifting, transfer techniques, and daily support.Adaptive Equipment Training: Teach patients how to use tools like shower chairs, reachers, or custom splints.Documentation: Keep accurate medical notes and track patient progress using agency software.Qualifications include:Master's or Doctoral degree in Occupational Therapy from an accredited program.Minimum of 1 to 2 years of clinical experience; home health experience is strongly preferred.Active state occupational therapy license and registration with the National Board for Certification in Occupational Therapy (NBCOT).Effective written and verbal communication.Valid driver's license, reliable personal vehicle, and active auto insurance.US Citizen or Green Card holder.Ability to bend, stand, walk, and assist with lifting or transferring patients (often up to 50 lbs or more).
