Physician Assistant - Assisted Living Visits

Overview Twin Cities Physicians is seeking a Physician Assistant (PA-C) to join our care team in Golden Valley and across the Twin Cities Metro. In this role, you’ll provide on-site, high-impact care to older adults in assisted living, memory care, and other senior housing settings. This is not a clinic-based job—it’s a flexible, autonomous role where you build real relationships with patients. Responsibilities Drive to senior living communities to provide on-site primary care to residents Facilitate the patient’s plan of care: chronic care management, advanced treatment, and end-of-life planning Coordinate with the interdisciplinary team, reconcile medications, and order labs Communicate clearly and regularly with physicians, support staff, facility teams, and families Qualifications Current registration as a Physician Assistant / PA by the MN Board of Medical Practice Strong portfolio in geriatrics, primary care, or internal medicine Passion for serving the older adult population Strong interpersonal communication skills Valid driver’s license and current auto insurance Military Occupational Classification (MOC) Codes If you’re a military member transitioning to civilian life, we value your service and skills.
