As a therapist within our organization, you will report to the Home Health Branch Administration and play a crucial role in assisting patients to regain their strength, mobility, and independence—all from the comfort of their own homes. By providing tailored care that emphasizes rehabilitation and functional enhancement, you will empower individuals to overcome physical challenges, confidently engage in daily activities, and improve their overall quality of life.

Position: Home Health Occupational Therapist

In this role, your responsibilities will include:

• Evaluating patients' daily living and work-related skills, and creating therapeutic retraining programs with clear, measurable goals.
• Administering and interpreting diagnostic tests to assess patients' functional abilities in their homes or other environments, aiding physicians in evaluating their level of function.
• Collaborating with the patient's physician and other healthcare professionals to develop or revise the Plan of Care Treatment.
• Delivering therapeutic treatments and guidance to patients as per physician directives to enhance strength, coordination, range of motion, and overall function, or teaching compensatory strategies.
• Adjusting retraining programs in alignment with each patient's behavioral goals and the overall rehabilitation framework.
• Consulting with vocational team members to create and implement vocationally focused plans that meet the needs and abilities of patients.
• Updating the physician, rehabilitation staff, Clinical Manager, and family members on patient status and progress.
• Engaging in care coordination and discharge planning, serving as a resource for other healthcare personnel to address patient needs.
• Designing community reintegration activities to support clients in their physical recovery and psychological adjustment, coordinating efforts with the interdisciplinary team.
• Recommending or creating specialized adaptive equipment to enhance clients' home or work environments and facilitate their participation in rehabilitation and community activities.
• Training staff and family members to implement the retraining program, including skills related to dressing, feeding, grooming, and hygiene, and participating in educational meetings with the team and family.
• Supervising and evaluating the performance of Occupational Therapy Assistants and Home Health Aides in delivering occupational therapy services.
• Documenting patient care observations, interventions, and evaluations accurately and thoroughly on the day services are provided. Ensuring that interim verbal orders from physicians are documented and executed properly. Submitting evaluations, treatment plans, and discharge summaries to supervisors and care management staff.

Utilize Your Skills to Make a DifferenceRequired Qualifications:

• Active and unrestricted Occupational Therapy licensure.
• Current CPR certification.
• Valid driver's license, auto insurance, and dependable transportation.

Preferred Qualifications:

• At least six months of experience in occupational therapy.
• Experience in home health settings.

Compensation and Benefits

Pay Per Visit/Unit Rate: $49.81 - $68.46

Scheduled Weekly Hours: 1

The compensation range provided reflects a good faith estimate of the starting base salary for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.

Annual Salary Range: $76,900 - $105,800
