The Role As a Nurse Practitioner at Greater Good Health, you will provide exceptional care to senior Medicare patients, addressing their needs and concerns. Support highly complex patients, assisting them in navigating a challenging healthcare system and providing one-on-one care they need and deserve Provide patient education, develop dedicated care plans, and provide clinical guidance to an integrated care team with patient-centered principles Work closely with the Integrated Care team to provide superior care for our dialysis patient population Function autonomously and collaboratively with all healthcare team members to coordinate and facilitate high-quality, cost-effective care Coordinate care services among patient, family, physicians, additional providers, and other care team members Administer various tests and screenings Conduct Comprehensive Health Assessments and Health Risk Assessments, requiring a holistic view of health and accurate, specific clinical coding and documentation Manage longitudinal care of an assigned patient panel of CKD and ESKD patients, focusing on management of conditions, timely post‑discharge follow‑up, utilization management, prevention of readmissions, and medication management Maintain strict confidentiality in line with HIPAA regulations What You Can Expect This full-time Nurse Practitioner position includes a mix of in-clinic (dialysis centers and nephrology practices) and home visits primarily within a designated service area, with occasional telehealth visits. The role may require flexibility across regions to provide coverage as needed.
