What You'll Do:

• Monitor residents' physical and mental well-being; communicate observations to the team
• Receive, transcribe, and implement physician orders accurately
• Administer medications and treatments as prescribed, document accordingly
• Supervise and support LNA staff, ensuring quality resident care
• Assist physicians during resident rounds and 60-day reviews
• Provide oxygen therapy, wound care, and manage gastrostomy/jejunostomy feedings
• Perform daily unit dutieschecking appointments, monitoring vital signs, ensuring infection control, and maintaining a safe environment
• Complete admissions, discharges, and transfers; update documentation
• Communicate regularly with residents' families and coordinate with staff
• Manage resident care during your shift, under supervision, following regulatory standards
• Support new staff orientation and participate in ongoing education and training

What You Need:

• Valid nursing license (LVN/LPN) in New Hampshire or eligible for licensure
• Proven experience in long-term care or similar setting
• Strong communication and clinical skills
• Ability to work independently and as part of a team
