Job ID: P139210-1414 Responsibilities As a member of a multidisciplinary health care team, medical social workers provide psychosocial interventions to patients and families within any department of LPCHS. Assess social needs related to the patient’s health status and develop a treatment plan, including advocacy within the organization and community, counseling, referrals, and practical assistance. Provide consultation, orientation, and training to health care staff on biopsychosocial, cultural, and economic issues affecting patient care. Advocate for social work core values and ethics within Stanford Medicine and the broader community. Titles Licensed Clinical Social Worker (LCSW) : Fully-qualified level of professional social work for licensed practitioners in California. Social Work Clinician : Entry/developmental level for recent graduates or unlicensed social workers. Essential Functions Perform psychosocial assessments of patients and families to evaluate social, emotional, and physical needs; identify barriers to care. Collaborate with interdisciplinary teams to create and implement psychosocial treatment plans; coordinate referrals to specialty and social services. Provide supportive counseling, grief counseling, crisis intervention, and psychoeducation to patients and families. Manage assigned caseloads and participate in team conferences and rounds. Prepare and maintain electronic medical records; provide written and oral reports and hand-offs. Minimum Qualifications Licensed Clinical Social Worker : Education: Master’s degree in Social Work from a CSWE-accredited program or equivalent (as demonstrated by licensure). Experience: Two (2) years of progressively responsible related work experience. License/Certification: Valid LCSW license from the California Board of Behavioral Sciences. Social Work Clinician : Education: Master’s degree in Social Work from a CSWE-accredited program; successful completion prior to start date. If MSW is not CSWE-accredited, a certificate verifying Associate Clinical Social Worker eligibility from BBS is required. Knowledge Ability to apply theories and practices of medical social work in a hospital setting. Ability to assess needs and manage caseloads with confidentiality and emergency action capabilities. Knowledge of hospital resources and cultural considerations relevant to LPCH’s patient population. Additional Information Positions in this job family are flexibly staffed from Social Work Clinician to Licensed Clinical Social Worker. A Social Work Clinician may be promoted to LCSW when qualified and recommended for promotion. Pay Range Compensation is based on the level and requirements of the role. Salary ranges may depend on education, experience, knowledge, and location.
