Skilled Nursing Facility Physician

Position Information: We have a team of 5 physician SNFists and 5 APPs as well as looking to grow to provide services for 3 post-acute rehabilitation facilities. That in itself is an incredible patient to provider ratio and an example of Lee Healths commitment to provide a Quality Based care approach! As a SNFist, you will dedicate your expertise to post-acute care environments. This role will allow you to address the specific needs of patients transitioning from hospital care to rehabilitation or long-term residency in SNFs (Skilled Nursing Facility) and SNUs (Skilled Nursing Unit). You will create a pivotal shift toward specialized, quality care tailored for a vulnerable segment of the population often in dire need of attentive, personalized medical oversight. You will also complete comprehensive medication reconciliation management and treatment plans of care to ultimately have a goal to return our patients back to their original home setting as quickly as possible.
