Internal Medicine Physician (MD/DO)

Location: Long Island, New York
Job Type: Full-Time

Position Overview

We are seeking a Board-Certified or Board-Eligible Internal Medicine Physician (MD/DO) to join a well-established outpatient primary care practice. This is a 100% outpatient opportunity with an established patient panel, strong clinical support, and a collaborative multidisciplinary team. No hospital rounding is required, and weekend call responsibilities are shared among providers.

Key Responsibilities

• Provide comprehensive primary and preventive care to adult patients.
• Evaluate, diagnose, and manage acute and chronic medical conditions, including hypertension, diabetes, chronic kidney disease (CKD), COPD, hyperlipidemia, thyroid disorders, and other internal medicine conditions.
• Perform annual physical examinations and Medicare Annual Wellness Visits.
• Order, review, and interpret laboratory tests, imaging studies, and other diagnostic procedures.
• Develop individualized treatment plans and manage medications.
• Educate patients on preventive care, chronic disease management, and healthy lifestyle practices.
• Coordinate referrals and collaborate with specialists and multidisciplinary healthcare providers.
• Conduct telehealth visits when appropriate.
• Maintain accurate documentation in the Electronic Health Record (EHR).
• Participate in quality improvement and value-based care initiatives.

Qualifications

• MD or DO degree from an accredited medical school.
• Board Certified or Board Eligible in Internal Medicine.
• Active New York Medical License (or eligibility to obtain).
• Active DEA Registration (or eligibility to obtain).
• Strong commitment to patient-centered, evidence-based care.
• Excellent communication and interpersonal skills.
