Impactful Hospice Intake & Care Coordinator
Hospice Buffalo
Hospice Buffalo is seeking a Patient Services Specialist (Intake) to coordinate intake, scheduling, and care delivery for patients. You will conduct interviews, gather demographics and insurance information, and refer to service providers as needed, under the supervision of the Admission Team Manager.
The role requires assisting in coordinating assessments, entering data into the EMR, and supporting daily department operations.
