Work Environment\n This role will primarily be conducted from a remote setting, allowing for flexibility and accommodating the needs of both the social worker and the patients. Despite the remote nature of the position, regular communication and collaboration are emphasized within our interdisciplinary rehabilitation team to ensure the highest standard of care. Regular business hours are observed, with some flexibility to accommodate patient needs, including occasional evening or weekend shifts as necessary. Key Responsibilities\n \n Conduct thorough psychosocial assessments through a combination of patient interviews, observations, and meticulous review of medical records to identify emotional, social, and environmental factors that may impact recovery. \n Provide individual counseling and family support, helping patients navigate the complexities of adjustment issues, including grief and coping mechanisms associated with recovery challenges. \n Collaborate closely with the rehabilitation team—including physicians, nurses, and occupational and physical therapists—to formulate individualized treatment plans tailored to the unique psychosocial needs of each patient. \n Assist patients in securing necessary community resources, such as support groups, vocational rehabilitation services, and housing assistance, to further their ability to reintegrate into society post-rehabilitation. \n Educate and empower patients and their families about available community resources and post-rehabilitation support services to enhance long-term success. \n Contribute to discharge planning processes and help facilitate smooth transitions for patients moving from detox settings to home or other care facilities. \n Patient Population / Client Focus\n The primary client population consists of individuals undergoing detoxification and rehabilitation that face various emotional and psychological challenges related to their recovery. These patients may be dealing with substance use disorders or mental health issues that complicate their rehabilitation journey. Our goal is to address their holistic needs and aid them in achieving sustainable independence and health. Qualifications\n \n Master's degree in Social Work from an accredited program. \n Active state licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey. \n Strong grasp of adjustment issues, grief processes, and effective coping mechanisms as they pertain to patient recovery. \n Exceptional communication and interpersonal skills for effective patient interaction and teamwork. \n Preferred Experience\n \n Experience in a medical or rehabilitation setting is preferred, particularly with clients who have substance use disorders and mental health challenges. \n Familiarity with therapeutic modalities useful in the context of social work and rehabilitation. \n Proven ability to work collaboratively within a multidisciplinary team environment. \n Compensation and Benefits\n The position offers a competitive salary ranging from $75,000 to $100,000 annually, along with opportunities for performance-based bonuses and a sign-on bonus.
