Hospice Admission Nurse

Hartford Healthcare is seeking a compassionate Hospice Admission Nurse to coordinate and complete hospice admissions for patients and families across our care network. In this role, you will conduct comprehensive nursing assessments, verify eligibility for hospice services, develop initial plans of care, and educate patients and caregivers on hospice philosophy, services, and symptom management. You will collaborate closely with physicians, social workers, and the interdisciplinary team to ensure a seamless transition to hospice and timely initiation of services.
