Certifications/Licenses:

• Pelvic Health Certification Required.
• Current, active Physical Therapist licensure for Colorado or a permit from the licensing board while awaiting licensure examination.
• Current CPR Certification.

Mandatory Experience:

• Minimum 4 years experience in Pelvic Health required.
• Physical Therapist must have completed training in accordance with minimum essential for physical therapy schools as prescribed by Council on Medical Education and Hospitals of American Medical Association.
• Graduated from an AMA-APTA approved school of physical therapy.
• Member or eligible for membership in the American Physical Therapy Association or American Registry of Physical Therapy
• Active license for the state of Colorado.
