What You’ll Do As an ADvantage Nurse LPN, you support patient independence and safety through high‑quality clinical care. Working under the supervision of an RN, you will: Complete initial assessments, nursing evaluations, admission paperwork, and task sheets Educate patients and families on services, policies, fall prevention, and care needs Coordinate with members and case managers to ensure appropriate care plans Provide authorized clinical services such as medication preparation, skin monitoring, and nail care for qualifying members Maintain strict infection control practices Document all care in accordance with organizational standards Reassess patient needs regularly and adjust care plans as needed Ensure compliance with all regulations and uphold confidentiality at all times This role is both challenging and deeply rewarding-you'll make a meaningful difference in the lives of patients and families every day. Qualifications Active LPN license with at least six months of monitoring‑visit experience CPR certification within 60 days of hire Valid driver's license, clean driving record, reliable transportation, and proof of insurance Ability to travel long distances and adapt to frequent schedule changes Need to be able lift, push, pull and carry up to 50lbs. Need to be able lift, push, pull and carry up to 20lbs 70% of the time and 30lbs up to 25% of the time.
