Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Title Physical Therapist Location New Jersey Schedule / Job Type Contract | Days | Outpatient | Onsite Key Responsibilities Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction, utilizing 60-minute evaluation and treatment sessions Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations Adhere to discipline-specific State Practice Act and all regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements Required Qualifications Active and unrestricted Physical Therapist license in the state of New Jersey Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans Proficiency in medical record documentation in compliance with state, federal, and payer guidelines Knowledge of and adherence to discipline-specific professional code of conduct and State Practice Act Commitment to patient and employee safety standards, including safe patient handling and infection control practices Preferred Experience / Skills Prior experience in an outpatient physical therapy setting Experience supervising or mentoring physical therapy students Active involvement in continuing education and professional development activities Why Join Us Work in a structured outpatient environment that emphasizes quality patient care, professional growth, and a collaborative team culture Opportunity to develop an individualized professional development plan and pursue continuing education within a supportive clinical setting Concentric Healthcare Staffing Job ID 765590.
