Date Published: 03/24/2024 REPORTING RELATIONSHIPS Reports to: Pharmacy Manager GENERAL SUMMARY Works under the supervision of the Pharmacy Manager. The primary job function of the Registered Pharmacist is to distribute medications and supplies according to departmental procedures with the assistance of pharmacy technicians. Collaborates with physicians and other hospital staff on medication related matters. Educates patients and their families to optimize drug therapy. QUALIFICATIONS Education: High School Diploma or successful completion of an equivalent High School Exam Required Bachelor's degree in Pharmacy (5 year) is required Doctor of Pharmacy (Pharm D) is also preferred Licensure: Pharmacy(OH) license required Graduate Pharmacists may also be hired awaiting license BLS required (if assigned to Anticoagulation Clinic only) Experience: Previous hospital experience preferred Interpersonal Skills: Must be able to communicate effectively with hospital, physicians, and nursing personnel when clarifying medication orders and dispensing pharmaceutical supplies. Must be able to provide drug information to a variety of clinical personnel. Must assign work and provide guidance to departmental personnel. Essential Technical/Motor Skills: Inputting data/typing, speaking clearly, answering telephones, precise eye/hand/foot coordination, fingering/fine dexterity, delicate/precise manipulation, handling/gripping. Essential Physical Requirements: Lifting/transferring/carrying light (20 lbs. Maximum). Pushing, pulling, squatting, kneeling, reaching, positioning, standing, holding, walking repetitive activities. Essential Mental Requirements: Calculations, manipulating numbers, interpreting numbers, analyzing, forecasting, assessing/evaluating, explaining/teaching, synthesizing, attention to detail, spatial/form perception, memory, problem-solving/reasoning. Essential Sensory Requirements: Ability to see, hear, feel, taste/smell, distinguish color. Exposure to Hazards: Various chemicals, electrical apparatus, mechanical apparatus. Eye strain from detailed work and computer operation. OSHA category I - tasks that involve exposure to blood, body fluids or tissues. Other: N/A JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS Compounds and dispenses medications and pharmaceutical supplies according to accepted standards of practice to fill written prescriptions issued by qualified prescribers. Checks the accuracy and appropriateness of all medications dispensed. Intervenes clinically with the physician when necessary and appropriate to optimize drug therapy. Maintains accurate and appropriate patient profiling and charging records for all inpatient and non-inpatient medication orders. Follows accepted standards of practice and adheres to federal, state, and local regulations, as well as the policies and procedures of SOMC and Pharmacy Services. Serves as a drug information source for patients, physicians, nursing personnel, hospital staff, visitors and the community. Assists the Director of Pharmacy with certain assigned reports or projects. Maintains a level of professional competency and remains current in the field of pharmacy. Assigns, teaches, supervises, and monitors the work of Pharmacy Technicians, students, interns and externs. For clinical pharmacists, the pharmacist must serve as a preceptor for 2 students in a calendar year. (as it applies) For clinical pharmacists, the pharmacist must present one clinical and one anticoagulation update in a calendar year. Demonstrates the ability to accurately compound hazardous medications in accordance with USP 800 and NIOSH guidelines. Method of Assessment: DO, or V Population Served: AA Checks & Audits: Demonstrates the ability to check accuracy and appropriateness of all medications are dispensed in accordance with state and federal law. Also, checks orders to ensure proper identification (2 methods) for physician signature, dose, frequency, route of administration. Method of Assessment: E/T, or T Population Served: AA Calculations: Makes appropriate calculations when determining doses for infant, children, adolescent, adults, and geriatric patients. Method of Assessment: E/T, or T Population Served: AA Special Projects: Assists the Director of Pharmacy with certain assigned reports or projects.
