Seeking an experienced Registered Nurse Case Manager for a 14-week travel nursing job in Randallstown, Maryland . This on-site hospital case management opportunity supports emergency department and medical inpatient coverage, with a focus on care coordination , discharge planning , and collaboration across interdisciplinary teams. • Weekly Estimated Pay: $1,775 - $1,972
• Location: Randallstown, Maryland
• Setting: Hospital-based case management with ED and medical inpatient coverage
• Start Date: 8/3/2026
• Duration: 14-week travel assignment
• Schedule: 40 hours per week
• Shifts per Week: 5
• Shift Duration: 8 Hour Days
• Shift Hours: 8:30 AM - 5:00 PM
• Weekend Rotation: Every 4th weekend, with additional weekend coverage possible based on staffing needs

Job Requirements

• Minimum 2 years of hospital inpatient med-surg and/or critical care discharge planning experience required
• Active Maryland RN license required and must be in hand prior to start
• Ability to work independently with a brief orientation and quickly adapt to unit workflow
• Strong care coordination and discharge planning skills
• Excellent communication and collaboration skills for partnership with case management and social work teams

Responsibilities

• Provide case management support for emergency department and medical inpatient populations
• Coordinate discharge planning to promote safe, timely, and effective patient transitions of care
• Collaborate closely with social workers, physicians, nursing staff, and interdisciplinary team members
• Manage assigned patient units, typically supporting approximately 20 beds in partnership with case management and social work colleagues
• Adjust coverage across additional units as needed during staffing fluctuations
• Assess patient needs, identify barriers to discharge, and help facilitate appropriate post-acute services and resources
• Maintain accurate, timely documentation and support quality patient outcomes throughout the care continuum
