Per Diem Home Health Occupational Therapist

Job Description Position Summary Provide home-based occupational therapy evaluations, treatment and discharge planning for adult and geriatric patients. Work on a per diem basis with flexible shifts; travel to patients' homes across Palm Beach County is required. Key Responsibilities Conduct skilled OT evaluations in patients' homes; develop measurable, reimbursable care plans Deliver skilled therapeutic interventions (ADLs, IADLs, upper-extremity function, home safety, mobility, splinting, adaptive equipment training) Complete timely, accurate documentation meeting Medicare, state, and agency standards Communicate with interdisciplinary team, physicians, and family/caregivers about progress and discharge needs Maintain caseload within the scope of practice and follow agency policies Qualifications Licensed Occupational Therapist (OTR or OTR/L) in the state of Florida Current BLS/CPR certification Minimum 1 year of clinical experience in home health, skilled nursing, outpatient or acute care Valid driver's license, reliable vehicle, current auto insurance, and willingness to travel throughout Palm Beach County Strong documentation skills and familiarity with home health Medicare regulations and OASIS preferred Excellent communication, clinical reasoning, and patient education skills Company Description At Allegiance Home Health & Rehab, our primary concern is the overall health, comfort and well-being of our clients.
