Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions and we are currently looking for a Physical Therapy Assistant (PTA) to join our dynamic home health care team.\u00a0

We are looking for a Physical Therapy Assistant (PTA) to join our team! You will utilize your clinical expertise to assess, teach, and deliver therapy interventions and treatment directed by the Physician as established in the home health plan of care. You will collaborate with interdisciplinary team members, receiving and providing pertinent updates to optimize patient well-being.\u00a0

Essential Job Functions: \u00a0

• \u00a0Provides direct physical therapy according to directions of physical therapist and in accordance with the physician\u2019s plan of care.\u00a0\u00a0
• Communicates with and regularly updates the patient and/or caregiver regarding the plan of care.\u00a0\u00a0
• Provides skilled therapy services, treatments and procedures as appropriate for the patient\u2019s care and safety.\u00a0\u00a0
• Reports observations of the patient\u2019s condition to the designated supervisor(s), and/or the patient\u2019s physician, as appropriate.\u00a0\u00a0

• Treats clients to relieve pain and/or develop or restore function to maintain maximum performance.\u00a0\u00a0
• Provides skilled teaching to the patients, family members, and caregivers regarding the safe use of prosthetic, orthotic and assistive devices (i.e., canes, walkers, wheelchairs, etc.)\u00a0\u00a0
• Educates patients and caregivers in carry over regarding issues concerning rehabilitation and adaptation techniques needed for recovery.\u00a0\u00a0
• Collaborates with other service disciplines to discuss the patient plan of care.\u00a0\u00a0
• Plans for client\u2019s discharge, prepares discharge instructions, and communicates discharge plan with patients, family members, and caregivers.\u00a0\u00a0

• Participates in in-service programs.\u00a0\u00a0
• Reacts to change productively and performs other job-related tasks and duties as assigned.\u00a0\u00a0
• Complies with ethical and professional standards of conduct as set forth by the American Physical Therapy Association\u00a0\u00a0
• Ensures confidentiality of patient records/information. Observes, records, and reports (1) signs and symptoms; (2) reactions to treatments and medications; and (3) any change in the patient\u2019s physical or emotional condition to the patient\u2019s physician and to the RN on the case if applicable, on a timely basis.\u00a0\u00a0
• Records clinical and progress notes at the time of the visit to ensure continuity of qualitative patient care on a timely basis.\u00a0\u00a0

• Completes daily report and any required documentation in a timely fashion to ensure agency operating schedules.\u00a0\u00a0
• Maintains consistent attendance.\u00a0\u00a0
• Submits all credentials in a timely manner.\u00a0\u00a0

Requirements:\u00a0\u00a0

• Graduation from a Physical Therapist Assistant education program from an accredited university/program.\u00a0\u00a0
• Current, unrestricted state license as a Physical Therapy Assistant in the state of practice\u00a0\u00a0
• Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures\u00a0\u00a0
