Physician (MD/DO) Skilled Nursing Facilities Full Time

About the Role We're looking for an experienced, autonomous Physician to join our clinical team full time, providing primary and episodic care toresidents in skilled nursing facilities. This is an independent role builtaround continuity you'll manage your own panel and build long-termrelationships with patients and their families. Rounds generally start in the morning(7–10 a.m.). No on-call, no overnights, ever. Chartfrom home. Your Day-to-Day You'll see roughly 16–25 patients a day a mixof long-term chronic care and acute issues. That means annual wellness visitsand H&Ps, managing chronic and co-morbid conditions, responding to acutechanges, reviewing meds and orders, and coordinating with collaboratingspecialists and facility staff to keep care seamless. You'll also stay in touchwith families and join care conferences where it makes sense. Qualifications MDor DO from an accredited medical school. BoardCertified (BC) required InternalMedicine, Family Medicine, or Geriatrics (or equivalent). Valid,unrestricted physician license in the state of operation. Mustbe opted-in with Medicare Successfulcompletion of an accredited residency program. Skillednursing or long-term care experience strongly preferred.
