Our team is growing and looking for a dedicated: Occupational Therapist (OT) - Home Health Shift Available: Full Time & PRN Service Areas for Coverage: Northeast, Southeast, Southwest Responsibilities:

• Facilitate rehabilitation of patients with emotional, physical, or mental disabilities.
• Develop and implement occupational therapy care plans.
• Coordinate care with home healthcare team.
• Evaluate therapy needs and monitor and report patient progress and outcomes.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications:

• Occupational therapist in the state of Texas.
• Home health experience highly preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Able to handle physical workload.
• Good driving record.
• Experience with HCHB EMR (preferred but not required).
• Effective communication and people skills.
