RN Care Coordinator (Case Manager)

Job Summary

The RN Care Coordinator (RNCC) is responsible for managing a caseload of patients, ensuring that their care aligns with desired clinical and financial outcomes. This role requires specialized skills and knowledge tailored to meet the unique needs of assigned patients. The RNCC coordinates care as per the guidelines set by an interdisciplinary team, leveraging patient assessments, care protocols, payer regulations, and responses to therapies to oversee the entire episode of illness from pre-admission through post-discharge.

Key Responsibilities

• Manage and coordinate care for a designated patient caseload, focusing on individualized patient needs.
• Utilize patient assessments and care guidelines to evaluate clinical outcomes and operational efficiencies.
• Implement care plans in collaboration with an interdisciplinary team to support optimal patient outcomes.
• Engage in ongoing evaluation of practice patterns and systems to enhance quality and satisfaction outcomes.
• Mobilize resources effectively to optimize the delivery of care and ensure resource efficiency.

Qualifications

• Current and valid RN license required.
• Experience in care coordination, case management, or related nursing practice preferred.
• Strong communication and interpersonal skills to interact effectively with patients, families, and healthcare teams.
• Knowledge of healthcare regulations, payer requirements, and quality standards.
• Ability to work both independently and as part of a collaborative team.
