RN Case Management

Location: Margate, Florida

Assignment Duration: This is a 13-week contract position.

Schedule: Shift: 8:00 AM - 4:30 PM

Job Description: The RN Case Management unit is responsible for managing patient care in an acute setting, focusing on coordination, collaboration, and communication among healthcare providers to ensure optimal patient outcomes.

Responsibilities:

• Manage patient care plans and coordinate services across healthcare providers.
• Work collaboratively with a multidisciplinary team, including social workers.
• Monitor patient progress and adjust care plans as necessary.

Required Experience / Certifications / Licensure:

• Must be a licensed Registered Nurse (RN) with experience in an acute care setting.
• Basic Life Support (BLS) certification required.
• ACLS certification is preferred.
