Mental Health Case Manager
Residential Transitions Inc
Residential Transitions is seeking a Targeted Case Manager in Minnesota to assess needs, set goals, and develop service plans for individuals with serious mental illness, substance use disorders, and co-occurring conditions. You will refer and link clients to needed services, monitor progress, and coordinate ongoing care in community and residential settings.
