Position Profile The Admission Coordinator develops and maintains relationships with referral sources and provides patient assessments in various settings to facilitate a smooth transition into Hospice of the Valley programs. Care delivery primarily occurs in hospital settings and skilled nursing facilities. Responsibilities Facilitates smooth transition and admission of patients from health care settings into HOV programs. Promotes HOV services and programs. Develops and maintains therapeutic relationships. Plans care and intervenes effectively. Provides effective patient/family/caregiver teaching. Supports the death/dying process.
