DMH Care Manager

The Department of Mental Health is seeking a licensed mental health professional to join its adult Care Management team. This role collaborates closely with inpatient psychiatric providers, outpatient providers, and statewide Intensive Residential Recovery programs to oversee the clinical treatment of individuals under DMH care and custody. Responsibilities Coordinate treatment plans for individuals who are involuntarily hospitalized or in the community under an Order of Non-Hospitalization. Participate in regular conference calls and individual team meetings to address specific clinical needs. Maintain a strengths-based, recovery-oriented framework to support teams in delivering care in the least restrictive setting. Execute prior authorization and concurrent reviews for Intensive Residential Recovery level of treatment, authorizing Medicaid funding. Complete administrative tracking and documentation, and manage independent task and time-management tasks. Provide coverage for reviewing involuntary hospitalization paperwork and making referrals to inpatient psychiatric hospitals when statutory criteria are met. Communicate closely with the DMH Legal team, Emergency Departments, inpatient psychiatric hospitals, Designated Agency Emergency Services teams, and the Department of Corrections. Develop and maintain strong collaborative relationships with community partners and support problem-solving in complex situations. Minimum Qualifications Master’s degree in a mental health related field. Four (4) years of clinical experience in counseling, social work, psychology, or nursing, including experience with adults with severe emotional disturbance. At least two (2) years of experience in a community-based system of care. Preferred Qualifications Knowledge of treatment options, including inpatient treatment for adults. Special Requirements Currently licensed mental health professional (Licensed Clinical Social Worker, Licensed Psychologist, Licensed Clinical Mental Health Counselor, or Licensed Alcohol and Drug Counselor) or possession of a Vermont license within six months of hire. Compensation and Benefits As of July 12, 2026, the step 1 rate for pay grade 26 is $46.78 per hour and the step 15 rate is $65.80 per hour. 80% State-paid medical premium and a dental plan at no cost for employees and their families. 11 paid holidays and a generous leave plan.
