Home Health Physical Therapist (Hybrid)

Position Summary

We are seeking a licensed Physical Therapist (PT) to provide home health therapy services across our growing coverage zones. This position offers a premium starting rate of $95.00 per session , with the flexibility to choose a geographic territory that aligns best with your location and commuting preferences.

Coverage Area & Territories

We are actively hiring to cover the following regions. You can customize your territory based on the cities listed below:

• North Orange County:  Anaheim, Fullerton, Brea, Buena Park, Yorba Linda, Placentia, Cypress, La Habra, La Palma, Los Alamitos, and Stanton.

• Central Orange County: Santa Ana, Orange, Tustin, Costa Mesa, Fountain Valley, Garden Grove, Westminster, and Villa Park.

• South Orange County: Irvine, Mission Viejo, Lake Forest, Aliso Viejo, Laguna Hills, Laguna Niguel, Rancho Santa Margarita, San Juan Capistrano, and Ladera Ranch.

Key Responsibilities

• Conduct comprehensive initial assessments, developments, and implementation of individualized physical therapy plans of care.

• Provide functional mobility training, therapeutic exercise instruction, gait training, balance training, and pain management techniques in patients' homes.

• Educate patients, family members, and caregivers on home safety, fall prevention, and home exercise programs.

• Document clinical notes timely and accurately using our home health electronic medical records (EMR) software.

• Collaborate effectively with physician teams and our internal clinical coordinators to ensure seamless continuity of care.

Qualifications & Requirements

• Active, unencumbered license as a Physical Therapist (PT) in the state of California.

• Graduate of an accredited Physical Therapy program.

• Prior experience in Home Health Care is highly preferred (experience with OASIS documentation is a plus, but we are open to training the right clinical candidate).

• Current CPR / BLS Certification.

• Valid California Driver’s License, reliable transportation, and active auto insurance.

• Excellent communication skills, clinical judgment, and time management.
