Key Responsibilities

• Provide physical therapy services to patients in a home health setting
• Complete and manage OASIS assessments as required for home health documentation
• Develop and implement individualized treatment plans to support patient recovery and functional goals
• Travel to patient homes within the assigned service area; mileage reimbursed at the IRS rate
• Maintain accurate and timely clinical documentation
• Float to other locations as needed based on operational needs

Required Qualifications

• Active Physical Therapist license; Virginia license required to be in hand at time of submittal or valid compact license with proof provided
• Minimum 6 months to 1 year of recent home health experience required
• OASIS experience required
• Specialty experience within the last year required
• One supervisory reference required at time of submittal
• Local candidates are welcome and accepted

Preferred Experience / Skills

• Experience working in a travel or contract therapy role
• Strong time management and ability to work independently in a home-based care environment
• Familiarity with electronic medical records and clinical documentation platforms
