About the Position

Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: Physical Therapy Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 6 months to 1 year of recent home health experience License: Valid physical therapy license required Certifications: ACLS, BLS, and PALS from the American Heart Association required Must-Have: OASIS experience required Must have a minimum of 6 months to 1 year of recent home health experience Description: The position involves providing physical therapy services in a home health setting. Candidates should possess recent experience in home health care. Familiarity with OASIS documentation is required. Mileage will be reimbursed at the IRS rate. Locals are welcome to apply. Candidates should review housing options prior to submission. Candidates must provide one supervisory reference for submission. All managers will conduct interviews. Floating may be requested based on facility needs. All units require specialty experience within the last year.
