What You'll Do

Evaluate and develop individualized treatment plans
Provide evidence-based physical therapy services to adult and geriatric patients
Help patients regain strength, mobility, balance, and independence
Collaborate with PTAs, OTs, OTAs, SLPs, nursing, and physicians
Complete timely documentation in accordance with Medicare and regulatory guidelines
Participate in interdisciplinary care planning and discharge planning
Deliver outstanding patient-centered care while achieving positive clinical outcomes

Qualifications

Graduate of an accredited Physical Therapy program
Current Michigan Physical Therapist license (or eligible for licensure)
Strong communication and clinical skills
Experience in Skilled Nursing, Post-Acute Care, Inpatient Rehab, Home Health, or Outpatient is preferred but not required
New graduates are encouraged to apply
