In-Home CKD Care Manager RN

Monogram Health is seeking a Registered Nurse Care Manager to partner with physicians and social workers in delivering in-home CKD/ESRD care. You will manage care plans, perform visits, educate patients, and support families to improve quality of life. Responsibilities include biometrics monitoring, medication reconciliation, and coordinating with dialysis providers.
