Home Care RN Case Manager

Community Health Network has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer a stable, supportive, and growing organization that focuses on your needs and interests. Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager. Coverage needs primarily in and around the Beverly/Marblehead/Salem/Swampscott, MA area.
- Full-time, Monday-Friday, 40 hours/week
- Occasional weekends may be required
- Highly competitive pay rates with IRS mileage reimbursement
